- Domicile Affidavit* -- Submit signed and dated only if you are claiming Arizona residency.
- Application fee of $65.00 payable by check or money order must accompany the mailed application. Fee effective 8/25/08.
If you are applying to a degree seeking program you will need to send separate mailings to the Graduate Admissions Office, and to each department to which you are seeking admission. Please verify the requirements of your major department using our Program Description Guide through our online catalog. In addition to this application form most departments have mandatory documents such as letters of recommendation, GRE or GMAT test scores, etc. that must be submitted. Failure to submit information in a timely manner may reduce your chances of acceptance
