AmeriCorps Forms/Links

In addition to being a Peace Corps Fellows/USA program, we are an AmeriCorps grantee through the Arizona Governor's Office, for our program, called Bringing Serivce Home. Many of our Peace Corps Fellows also become AmeriCorps Members as a means to provide service and fulfill their Peace Corps Fellow obligation. AmeriCorps Application Form - Please download, complete and submit to the Bringing Service Home Coordinator.

AmeriCorps Exit Form - Upon completion or termination of service, please download, complete and submit to the program coordinator. You will need to schedule an exit interview within 30 days of completing service.

Segal AmeriCorps Education Award - Upon successful completion of AmeriCorps service, you are eligible to receive a Segal Ed Award. This link provides information on how to access your award which can be used to repay student loans or for future tuition and fees for a seven year period.
