Forms
On this page you will
find a number of PDF forms that you can download, print out, fill out
and hand in. However, many forms (indeed the most commonly used forms)
need to be filled out online by logging in to our My GradColl system with your NetID
The purpose of having you fill out forms online is that, every time you click the "Generate" button at the bottom of a form, all of the information you filled in is saved in a database that we manage. This way, if something happens to your form between the time you print it out and you hand it in, you can easily re-generate a new one. Also, by filling out forms online, we can automatically fill in fields of the form that are repeated on multiple forms. For example, you can fill in your address just once, and that address will appear on all forms.
Our online form system is constantly in development, so if you have any problems please don't despair! Just contact the Graduate College and let us know.
